Question 1: Regarding CFCM: Can you explain how the process moves from the independent facilitator working to create CSP to an ISP involving an agency providing services?

Question 2: Some Support Coordinators who work for service provider agencies are taking the training to become independent plan writers. Won't that be a conflict of interest?

Question 3: We received a letter stating our case manager has retired. Who do we reach out to if we need support? We have not been reassigned.

Question 4: How can someone who's blind get a copy of their ISP written in braille?

Question 5: Will the S109 changes, which have been in place for last two years, apply to those using Self-Directed Services? Will they be receiving letters?
