Rhode Island’s 23rd Annual Statewide Self-Advocacy Conference!

2018 Self-Advocacy Awards


Each year, Advocates in Action asks people from throughout Rhode Island to nomiate Self-Advocates and Allies for 3 amazing Awards. Our Conference Planning Committee meets with previous recipients to select and notify the winners, and the awards are presented to each of them during a luncheon ceremony at the Annual Statewide Conference.

This year's nominations are due no later than Friday, October 5th, and the winners will given complimentary registrations to WHY NOT?!?, RI's 23rd Annual Statewide Self-Advocacy Conference, on Thursday, October 25th.

Please note that, although we appreciate positive self-esteem, people may not nominate themselves. Previous Award Winners, and current Advocates in Action RI Statewide Board members are also not eligible.

This is an electronic form, which means you can fill it in on your computer and save it to send by e-mail, or print for regular mail. Note: If you want to complete the form on your computer and save a copy for your records, you need to DOWNLOAD IT FIRST

When you click on the form, it will open in a new tab on your web browser. Depending on your set-up, you should be able to fill-in the form and print it through your web browswer, but you won't be able to save your information.

Once you've downloaded your form and completed it, click on the "Save As" button and rename the file using your Last Name and the type of award while saving.

For example change the name from "2018_SA_awards_nominationE-form.pdf" to "LastName, Broken Record.pdf"

The 2018 Self-Advocacy Awards are a great opportunity to tell some of the Leaders, Advocates and Allies who you know how much you appreciate them! Winners will receive a custom-made plaque with their name engraved on it, and a Complimentary Conference Day Registration. If the person you nominate wins, you'll get a free registration, too!

We also invite winners, and the person who nominated them, to join our Board President on stage during lunch, where each of you will have the opportunity to share a few words.

Of course, before we can select this year's winners, we need some nominees! Fortunately, there are a LOT of award-worthy Self-Advocates and Allies out there. Please contact us if you have any questions about how to nominate one of them, and make sure your form is in the mail (or e-mail) no later than Friday, October 5th.

If you are planning to register, please send us your Registration Form as soon as possible. If you, or the person you nominate, wins an award, we will happily refund you conference day registration fee! But please DON'T WAIT or you may MISS OUT!
